Our priority is to provide patients with the best possible outcome and experience when undergoing treatment. The first step in identifying key parts of your situation is obtaining the history of events and any studies related to your injury. Please try to bring any images (X-rays, CT scans, MRI, etc.) and their reports to your appointment. We also understand that everyone has different needs and expectations. Describing work demands, recreational activities, and activities of daily living will all contribute to us having a better understanding of shared expectations. We aim to deliver the least invasive methods to achieve your goals. Routinely we recommend over-the-counter medications and therapy before injections or surgery. Everyone’s situation is different, so ultimately it comes down to the discussion with your provider, the status of your condition, and the consideration of how different treatments will impact you to determine what is the best option for you.

  • -Photo ID
  • -Insurance Card or Insurance Card image
  • -DOI
  • -Actual study images (not just reports)
  • -List of Medications with dosage
  • -Preferred pharmacy location
  • -Optional – Uploading information in Privia via Patient Portal OR ZocDoc
